Outside parties often request written certification of our legal name and federal taxpayer identification number.  We typically fulfill these requests by providing the requester with a signed copy of IRS Form W-9, completed with UC's data.

Vendors and suppliers may download a copy of UC's Form W-9 by selecting the link above.  This copy does not contain any mailing address, because UC has many departments with many different mailing addresses.  Vendors should not make any additional entries on this document.

UC employees may download our Form W-9 by selecting the link above, and have the option of entering their departmental name and address on the appropriate lines.  UC employees may then choose to save a copy of that form onto their own computer for future use.
