Request UC Tax Form Copies
Need another copy of your UC tax form? You're in the right place! First, you'll need to know which form you're looking for.
Form 1098-T, Tuition Statement, is provided in January to each of our students, except those foreign visitors who are "nonresident aliens" for tax purposes. Nonresident alien students may still obtain a Form 1098-T upon request.
Students can now access their 2006 Form 1098-T online, but they must sign up for this access. For more information about our new Electronic 1098-T Access, click here.
Paper copies of 2006 and 2007 Forms 1098-T will not be available. The only way to obtain a duplicate copy of your 2006 or 2007 Form 1098-T is to sign up for Electronic 1098-T Access.
2007 Forms 1098-T will be available by January 10, 2008.
To receive a duplicate copy of your 2004 or 2005 Form 1098-T, send an e-mail to tax@uc.edu with your full name, Social Security Number or UC ID number, phone number(s), and whether you are a U.S. citizen. Also, please indicate whether you prefer delivery via e-mail to your Bearcat Online address or via U.S. Mail to an address actively appearing on your student record. We will issue your prior-year tax form within 10 business days.
Form 1042-S, Foreign Person's U.S. Source Income, is sent to nonresident alien students and others who received money from UC in certain situations. Not all foreign visitors will receive a Form 1042-S. To receive timely e-mail updates about Form 1042-S delivery, find out what to do if you get one, or get help with filing your U.S. tax return, please join the UC International Services listserv. 2007 Forms 1042-S will be mailed by January 31, 2008.
Form 1099-MISC, Miscellaneous Income, is sent in January to many UC vendors. To request a copy of yours or to ask related questions, please contact our Accounts Payable office at 513-556-6746. 2007 Forms 1099-MISC will be mailed by January 31, 2008.
Form W-2, Wage and Tax Statement, is issued to UC employees each January by our Human Resources Service Center. To request a duplicate copy of yours, please visit their W-2 Duplicate Request Form web page.
