Disclosure Statement to UC Students for 1098-T Access
Your Form 1098-T, Tuition Statement (hereinafter, “statement”), can be accessed by you online after you give your electronic consent in Catalyst. If you do not give your electronic consent, UC will furnish one paper copy of your statement by US Mail to your Mailing, Permanent, Home or Campus address (in that priority order) in our records.
Your electronic consent applies without end, until you withdraw it as noted below. If, after you give your electronic consent, you wish to obtain a paper copy of your statement, you can do so by sending a written request (including your full name and UCID number) for the paper copy to Director, Tax Compliance, University of Cincinnati, PO Box 210641, Cincinnati OH 45221-0641. Such request will not be treated as a withdrawal of your electronic consent.
You will need to have the free Adobe Reader software downloaded onto your computer to open your statement. You can obtain Adobe Reader by visiting www.adobe.com.
UC intends to continue making statements available online to our students. Your statement will be available online through at least October 17 of the year following the applicable tax year. Online access may be temporarily interrupted by events outside UC’s control.
Keep Your Contact Information Updated
You should update your contact information (address, phone, etc.) in Catalyst whenever you change it, including after you no longer attend UC. Doing so will help UC to deliver your statements successfully.
Withdrawal of Consent
After you have given your electronic consent, you may withdraw it by sending a written notice including your full name and UCID number to Director, Tax Compliance, University of Cincinnati, PO Box 210641, Cincinnati OH 45221-0641. UC will confirm your withdrawal and its effective date, in writing to the mailing address from which we received your notice of withdrawal. No withdrawal of consent will be applied retroactively.
Withdrawals of consent entered into the former One Stop 1098-T portal (through which pre-2016 tax forms are still available) after December 31, 2016 will have no effect on 2016 and subsequent Forms 1098-T.
