The personal training packet helps us to better assign a trainer to meet your needs and goals.  It also helps determine whether a medical evaluation would be required prior to exercise. When it is complete, please return the packet to Member Services or fax it to 513-556-0601, ATTN: Coordinator of Fitness and Wellness. Payment must be received before a trainer will be assigned.

It's important to know where you are, in order to know where you want to be. That's why your first session will consist of a baseline fitness assessment and detailed goal discussion. We'll help you set S.M.A.R.T. fitness goals that are realistic and attainable for you!
