Use the A-910 form to obtain official signatures
on all contracts; to obtain signatures on an attachment to the University
Purchase Requisition; and to receive approval for purchases exceeding the
authorized limits (see Variant Section). Authorization to enter into any and all
contracts to which the University is a party rests with the Board of Trustees.
Complete Sections 1 through 7. Have the form signed by the individual responsible for the contract, the Department or Unit Head, the Dean or Division Head and the Vice President.
The Vice President's office will determine the need for signatures by: Vice President for Research and University Dean of Advanced Studies, Universiy Architect, University Treasurer's Division Representative or Purchasing Representative, Director of Risk Management and Insurance, University Patent Officer. From this point, the A-910 is forwarded to the Office of General Counsel. Depending on the dollar value involved, General Counsel will either sign the contract or forward it to the President or Board of Trustees for signature.
