2010 BENEFITS ANNUAL ENROLLMENT
The 2010 Annual Enrollment period will begin October 26, 2009 and will end on December 2, 2009. The Human Resources Department is GOING GREEN this year and is not printing Choice Enrollment Workbooks. You will find downloadable versions of the Enrollment Guide by clicking on the appropriate employee group below. You can download the complete guide or individual sections.
Annual Enrollment is your opportunity to review your insurance and flexible spending account elections and make your choices for the coming year. The benefit plans you select will become effective on January 1, 2010 and will remain in effect for the 2010 calendar year. You cannot change you benefit elections until the next Annual Enrollment period unless you experience a qualifying status change (only changes consistent with the status change are permitted).
Action is NOT required if you: a) are not interested in enrolling in the Flexible Spending Accounts for 2010, b) are satisfied with your enrollment elections in Medical, Dental, Employee or Family Life Insurance, PAI and Long Term Disability, c) have no changes in your covered dependents, d) have no changes in your Spouse/Domestic Partner Surcharge election and e) have no changes to your life insurance beneficiaries.
Action IS REQUIRED if you: a) are interested in enrolling in the Flexible Spending Accounts for 2010 (re-enrollment is required each year), b) are not satisfied with your elections in Medical, Dental, Employee or Family Life Insurance, PAI or Long Term Disability coverage, 3) have changes in your covered dependents, d) have a change in your Spouse/Domestic Partner Surcharge election or e) wish to change your life insurance beneficiary.
You will make your benefit elections by using Employee Self Service (ESS). Click on the following link for PC instructions or Mac instructions for logging on to ESS. Click on the following for instructions on how to use ESS to update your personal information or make your Annual Enrollment benefit elections. If you do not have access to a computer at work or at home, computers will be available at the Benefits Fair on November 6, the HR Service Center in University Hall, or at any campus library. For technical support (log-on or password problems), call the UC Help Desk at 556-4357 Monday-Friday from 7 a.m. to 9 p.m. and Saturday-Sunday from 8 a.m. to 9 p.m.

Assist America provides emergency travel assistance. When traveling in a foreign country or 100 miles or more away from home, you and your eligible family members can get help in the event of a medical emergency. |
| Michelle's Law |
Michelle's Law allows the continuing of medical coverage for a college student whose serious illiness or injury would nessesitate a leave of absence or change in enrollment status. |
| Mental Health Parity |
The Mental Health Parity and Addiction Equity Act of 2009 requires group health plans that offer mental health or substance abuse benefits to provide those benefits 'on par' with medical and surgical benefits. This means that UC's medical plans have been modified to remove limitations on mental health and substance abuse treatment. Treatment pre-authorization will continue to be required. |

If you are paying the Surcharge and it will continue to apply, NO ACTION is required unless you are changing your Plan enrollment. If you are paying the Surcharge and it no longer applies to you effective January 1, 2010, you must update your Surcharge information online. |
| Coordination of Benefits |
You will not be required to fill out a form advising of other medical coverage; however, you may be required to provide this information to Humana. You will be contacted directly by Humana. |
| Qualified Status Changes |
You must notify the Human Resource Service Center (HRSC) within 31 days of a qualified status change (e.g., birth, marriage) in order to make changes to your benefit elections. Once you have notified the HRSC, you will make your enrollment changes online. A paper form will not be accepted. You will be required to supply supporting documentation for many of these changes. |
| Accessing Provider Websites |
Employees can access the web sites of their medical, prescription and dental plans. Additionally, you can set up a personal account on Humana.com to find providers, view medical, prescription and dental claims history, find information about drugs including side effects, order replacement ID cards, complete an online health risk assessment and much more.
