Request for Duplicate Form W-2

Requests for duplicate 2007 Forms W-2 will be processed beginning February 11, 2008. Reissued W-2s can be printed during regular business hours, and requests submitted online will generally be available for mail delivery or for pick-up at the HR Service Center the next business day.

To request a duplicate Form W-2, please fill out the following, and click on "Send Form". To inquire whether an undeliverable Form W-2 has been returned to Payroll Operations, please call the Payroll Office at 556-2451.
