Welcome to the University of Cincinnati!  Successful completion of the New Hire Forms process will result in your data being entered in UC's HR/payroll system and enable us to provide you with your ID, computer access and your first scheduled paycheck.

Please bring the forms listed below in the Required Forms section with you on your first day of employment. You cannot begin working for the University until the new hire form process is complete.

• Form I-9, Employment Eligibility Verification (list of acceptable documents shown on page 5)
• Statement Concerning a Job Not Covered by Social Security - Form SSA 1945 (OPERS)
• Ohio Ethics Law and Related Statutes Acknowledgment
• Ohio Ethics Law and Related Statutes Acknowledgment Outline
• OPERS F-3 Request for Optional Exemption as a Student (Not Required)

• Direct Deposit Authorization Form
• Form IT-4 - Ohio State Tax Form
• Form WH4 Indiana State Tax Form
• Form K-4 Kentucky State Tax Form
• Form W-4 - Employees Withholding Certificate
