Predeparture Orientation

All UC students who participate in a study abroad program, or a UC-sponsored international experience, are required to attend a pre-departure orientation session and complete a set of emergency, health and liability forms.  Faculty Program Directors should communicate with UC International Programs to identify a time for this pre-departure session, usually one month prior to the date of departure.  International Programs staff members will present general information about study and travel abroad from the booklet Know Before You Go, and collect student forms, which are completed in duplicate.  One set of paperwork will travel with the Program Director while the other remains on file with UC International.  It is crucial to have these forms completed by all participants in the case of a medical emergency.

Statement of Responsibility and Authorization to Participate in the University of Cincinnati Education Abroad Program.
* Emergency Information Form (34.5 KB)
The information requested in this form will be used only in case of emergency and is limited to the duration of your participation in an international program. The information will be kept confidential and will be destroyed upon completion of the program.
* Health Insurance Form (36.4 KB)
All students must be covered by UC or other medical insurance.
* Emergency Treatment Authorization (65.0 KB)
The purpose of this form is to help UC International Programs provide appropriate assistance to you should the need arise during your study abroad experience.
* Time Away Form (55.6 KB)
You must fill out this form if you intend to spend time away from your program’s location, and your activity is not one already planned as a part of the program.
