Students: Opt-in to be eligible for on-campus COVID-19 vaccine opportunity

Dear UC Students:
In the interest of health and safety, the University of Cincinnati is partnering with UC Health to accept dedicated doses of COVID-19 vaccine specifically for students, which will be administered by UC Health at a vaccine clinic on UC’s campus.
This vaccine clinic offering the free.
The clinic is for those 18-years-old or above.
On Tuesday, April 13, student walk-ins (age 18 or above) are welcome from 8 a.m.-5 p.m. Students will receive the Pfizer vaccine. We will give students the option to come back to campus for the second dose (on May 5, at Fifth Third) or to get the second dose out of town.
The vaccine is provided by appointment that day as well.
Vaccination takes place in Fifth Third Arena.
If you want to expedite the process before you arrive, you can opt-in (link below) to provide consent that your data will be sent to UC Health.
In order to more easily allow students to register for and take advantage of this vaccine opportunity (while current supplies last), UC originally hoped to expedite data sharing with UC Health and sent all students a notice letter last week. However, given data privacy considerations, UC will now ask students interested in taking advantage of this vaccine opportunity to OPT IN and provide their consent to UC to share their necessary personal information with UC Health in order to create a patient account in UC Health's HIPAA protected electronic health record, Epic. The necessary personal information needed to create a patient account in Epic is: student name, M number, UC email address, phone number, permanent address, date-of-birth, gender, and Social Security Number. Students who do not provide UC with consent to share this personal information with UC Health will not be able to participate in this vaccine opportunity.

After you provide consent, your data will be sent to UC Health. You will then receive a UC Health invitation with additional information on next steps and how to schedule your on-campus vaccination.
