Employees: Don't Forget the Nov. 21 Deadline

This Thursday, Nov. 21, is the deadline for Choice Benefits Enrollment for thousands of UC faculty and staff. Completed enrollment forms for 2003 must be returned to the Benefits Office at Mail Location 0099. The hours of operation for the Benefits Office are 8 a.m.-5 p.m. Monday through Friday. A drop-off box is also located in the front entrance of University Hall.
UC faculty and staff should review the information contained in their Choice Benefits packets, the Choiceline newsletter or annual enrollment information at the Human Resources Web site. A summary of changes in this year's Choice Benefits Enrollment program is outlined on page 2 of the workbook.
When completing the enrollment form do not forget to complete sections "C," "D" and "E" on the back of the form. If you are waiving coverage you must submit a completed coordination of benefits form with your enrollment form for verification of other coverage. If you or any of your covered dependents have other medical coverage, you must also complete the coordination of benefits form.
UC employees and faculty who did not receive a Choice Benefits enrollment form should contact the Benefits Office immediately at (513) 556-6381.
