Waiving Student Health Insurance

University rules require all undergraduate, co-op, and graduate students who register for six (6) or more credit hours to be covered by health insurance. All students who register for six (6) or more credit hours will be assessed single student coverage unless they have previously waived coverage during the current academic year. Students who have been assessed insurance charges will be required to pay the charges unless a complete and accurate Insurance Waiver Form/Web Waiver is received in the Student Health Insurance Office by the third Friday of the term/semester.

  • To waive coverage you must complete and submit an Insurance Waiver Form via the web, mail, fax, or bring it to the Student Insurance Office (room 112 Scioto Hall). If you mail it, use certified mail to ensure it is delivered and received by the Student Insurance Office.  Do not write a note to or call a UC office to waive your coverage. 
  • Make or print a copy of your waiver before you submit it. Record the date it was submitted. 
  • Call the Student Health Insurance Office before the deadline to waive to verify that the waiver was received and processed at 556-6868.  
  • The waiver deadline is the third Friday of the term or semester. If you submit a waiver for the fall term or semester, it remains in effect the entire academic year. If you do not waive the Student Health Insurance, you are automatically covered by the Plan and you will be charged for the coverage. If you ignore the waiver deadlines, you will be responsible for the insurance charge.   
  • International students holding F or J Visas registered for one or more credit hours are required by the University to have health insurance coverage and will be assessed single student coverage unless a waiver is received in the Student Health Insurance Office by the third Friday of the term. 
  • Waivers will be accepted only from a US based insurance company with a US based claims company; coverage must be equal to or greater than the coverage offered by the University.
