Thank you for your continued participation in the process of developing program assessment plans.

It is now possible to request an extension for the May 15th deadline of submitting the program assessment planning documents and completed P-2 forms.  Please submit extension requests by April 15^th.  For your convenience, this request can be made in one of two ways:

1. Individual Extension Requests:  Program faculty can submit an individual extension request for each program (by PASLA code) using the below form.
2. Batch Extension Requests:  If this process is managed centrally by the college, an extension request for multiple PASLAs can be made by completing this spreadsheet and returning it to Program.Assessment@uc.edu.  If at all possible, it would be most helpful if we receive no more than 1 spreadsheet from each college.

We recognize that this process raises more of a challenge for some colleges than others, but want to assure you we are making every effort to support your endeavors. This form was created so that Programs can request an extension to the P-2 submission deadline of May 15th, 2013.  

Once you submit this form, you will receive a confirmation e-mail which will mean we have received your request.  Should you have any difficulties using this form, please reach out to our team at (Program.Assessment@uc.edu).

The extension requests will be reviewed in April and you can anticipate receiving a response to this request by mid-April.
