The personal training packet helps us to better assign your trainer to meet your needs and background. It also helps us to better understand whether a medical evaluation would be required prior to exercise. When it is complete, please return the packet to Member Services or fax it to 513-556-0601, ATTN: Coordinator of Fitness and Wellness. Payment must be received before a trainer will be assigned.
