Welcome to the UC Disability Services Office. Working together, we can put students first and ensure that they all have the best opportunity for success.  Our office provides accommodations to UC students. If you are a faculty member seeking accommodations for yourself, please contact George Wharton, Director, Office of Equal Opportunity.

Some instructors have asked us about information they can include on their syllabus to alert students to our services. Here is a sample statement:

Special Needs Policy: If you have a disability (e.g., visual impairment, hearing impairment, physical impairment, communication disorder, and/or specific learning disability) which may influence your performance in this course, you must meet with the Disability Services Office (DSO) to arrange for reasonable accommodations to ensure an equitable opportunity to meet all the requirements of this course. If you require accommodations due to disability, please contact DSO at 556-6823, Campus Location: 210 University Pavilion. You will be provided an Accommodation Form indicating your accommodation needs for the quarter. Please present this Form to me AS SOON AS POSSIBLE to ensure your accommodation needs are discussed, agreed upon, and provided.

Testing Procedures

One of the most frequently used accommodations our office provides is on-site testing which allows students to test in a quieter environment with fewer distractions and gives students the recommended time for the tests as supported by their documentation. If one of your students is taking a test in our office, there are a few procedures that will help ensure the integrity of the testing process and expedite the return of the test to you.

To ensure the validity of all tests taken in our office, we use test envelopes. On the day of an exam, a student will stop by our office to pick up an envelope to give to you. You should put the test in the envelope, seal it, and write your name across the seal. After you fill out the necessary information on the front of the envelope, the student will return to our office where we will open the envelope and administer the test.
When the student is finished, we will place the completed exam in a new envelope and fill out the front, marking the beginning and ending exam times. Once we seal the envelope, the test will be returned to you the way you have indicated on the original envelope. If no return instructions are given, we send the test with the student so he can return the test to your office or to the classroom.

Each envelope has instructions to assist you in filling out the necessary information. If you ever have any questions while filling out the envelope, you can contact us at 513 556-6823. Some professors prefer instead to drop their tests off personally at 210 University Pavilion or to e-mail their tests to our office. Either way, you need to give clear instructions about the test and its return so there are no miscommunications and/or unnecessary delays in the process.
