Registration Fee: $20 for an individual project, $30 for teams. Make checks out to the University of Cincinnati.

Abstract - Please submit a copy of your abstract (250 words maximum)

Form 1: Checklist for Adult Sponsor (Note: Date of Review must be prior to experimentation.)

Form 1A: Student Checklist (Both individual and team forms are provided here)

Form 1B: Approval Form (SRC/IRB must complete Section 2 if required for this project. Leave Section 3 blank.)

Form 1C: Research was conducted at a registered research institute

Form 2: This project was supervised by a qualified scientist

Form 3: This project uses hazardous chemicals, activities or devices, or regulated substances

Form 4: This project involves human subjects. (Check this box even if the project involves only a survey.)

Form 5A: This project involves non-human vertebrate animals

Form 6A: This project involves potentially hazardous biological agents

Form 6B: This project involves human tissue or non-human vertebrate animal tissue

Form 7: This project is a continuation of a previous year's project
