Services provided:  Evaluations and medication for anxiety, depression, grief,
bipolar disorders, schizophrenia, and personality disorders.  Available only to
students with UC Student Health Insurance. 
Names of providers: David Leonard, MD and 4th year Psychiatry residents
Types of Providers:  Licensed psychiatrist and 4th year psychiatry residents
Hours:  Monday – Friday 8:30 AM – 4:00 PM
Location: 3333 Richard E. Lindner Center
Cost:  $80 for initial visit; $70 for 25 minute counseling sessions.  UC Student Health Insurance pays $48.  Co-pay of $32 for full visit or $22 for half visit.

Services provided:  Individual, group, couple counseling, medication evaluations
and management.  Available to all UC Students

psychiatric nursing providers.    
Hours:  Monday – Thursday 8:00 AM – 8:00 PM; Friday 8:00 AM – 7:00 PM;
Saturday 9:00  AM - noon

Cost:  Sliding scale based on income, decided at intake appointment.

Services provided:  Individual, couple counseling, psychological testing,
medication evaluations and management.  Available to all students and management.  Available to all UC Students

psychiatric nursing providers.    
Hours:  Monday – Thursday 8:00 AM – 8:00 PM; Friday 8:00 AM – 7:00 PM
Location: 222 Piedmont Avenue, Suite 8500
Cost:  Full fee.  For a student with UC Student Health Insurance, UC Student
Health Insurance will pay $48 per encounter only if the student is referred from
UHS.  The student is responsible for all additional charges.  For a student with
other insurance, call 475-8710 to find out if your insurance is accepted.
