Management Referral Form

Supervisors can request services for individual staff or groups of people. Services can be suggested to employees or mandated as part of employee evaluations. ALL services are confidential but supervisors can contact REACH staff and request notice if an employee has attended an appointment. A release of information can be signed by the client if he/she agrees to further communication between REACH staff and supervisor.
