WORK RELATED INJURY

Worker's Compensation: Work-Related Injury and Illness
AT THE TIME OF INJURY OR ILLNESS

  1. Notify your Supervisor immediately !
  2. Report to University Health Services (UHS) for medical treatment during business hours or the closest emergency room after hours.
  3. Inform the medical provider at the time of treatment that this is a work-related injury.
  4. Complete a UC accident report [A-1352(a)] with your supervisor within 24 hours.
  5. Call UC Benefits at 556-OUCH (556-6824) for worker's compensation information.

The University of Cincinnati is committed to preventing work-related injuries and illnesses. Employees or students with life threatening injuries or illness should call 911 for immediate transport to University Hospital or other close hospital.

If the injury or illness is less severe and it occurs during the day 8:00a.m.- 4:00p.m., employees are encouraged to be seen at the University Health Services clinic on the first floor of the Homes building on the Academic Health Center side of Uptown campus. If the injury or illness occurs after hours, on the weekend or on a university holiday, employees are encouraged to get medical care from their personal physician or an Emergency Department.

Students with UC Student Health Insurance who have a less severe injury or illness are encouraged to be seen at University Health Services The Richard E. Lindner Center, 3rd Floor (513-556-2564) or Holmes (513-584-4457) clinics during the day or at University Hospital or other close hospital after hours and on weekends and university holidays.

Students with their own insurance are encouraged to be seen at the closest hospital Emergency Department or with a provider who is on their insurance plan.

Employees who are concerned that they might have become injured or ill in the course of their University of Cincinnati activities are responsible for reporting their injury or illness to the University within 24 hours by filling out an "initial Report on Work-related Injury or Illness" form [also called a 1352(a)] and sending it by campus mail to Environmental Health and Safety (ML 0218). Forms are available online at http:ehs.uc.edu/Accident/newform.asp  Employees have 24 months from the time of injury or illness to file an Ohio Bureau of Worker's Compensation First Report of an Injury, Occupational Disease or Death (FROI) claim.
