New Academic Program Creation and Current Academic Program Updates

Faculty interested in creating a new academic program will need to complete a New Academic Program Submission. Please read the instructions on this page to verify that the form is completed correctly. Forms submitted with incorrect or unclear data will be returned for clarification.


New Academic Program Instructions and Form Field Definitions

College

Name of college submitting request

Career

Career level of the program—e.g., Undergraduate, Graduate, Law, or Medicine (MD)

Academic Program Award

Credential the student will earn—e.g., Bachelor of Arts

Academic Program Plan

Plan/major of the program—e.g., History

Academic Program Plan Website Title

30 character maximum (includes spaces)—an abbreviated title must be provided for the University's program site if the Academic Program Plan is longer than 30 characters

Academic Program Plan Suggested Catalyst Code

A suggested code abbreviation must be provided in plan/major–award format—e.g., HIST–BA

Academic Program Sub–Plan

List the sub–plan/track of the program, if applicable—e.g., American History

Academic Program Sub–Plan Website Title

30 character maximum (includes spaces)—an abbreviated title must be provided for the University's program site if the Academic Program Sub–Plan is longer than 30 characters

Academic Program Sub–Plan Suggested Catalyst Code

A suggested code abbreviation must be provided in plan/major–sub-plan/track format—e.g., HIST–AH

CIP Code

Identify the six–digit code that best applies to the program from the IPEDS Classification of Instructional Programs site. The CIP code drives the classification of the program and degrees reported to ODHE and USDE.

Online Delivery

Distance–learning programs will automatically be assigned a sub–plan code in the *plan/major–DL format—e.g., *HIST–DL

Minimum Credit Hours

Indicate the minimum credit hours required to complete the program

Length of Program

Indicate the length of the program—in years. Must be a single digit; a range is not acceptable.

Begin Term

Indicate the first term that the program will be offered—e.g., Fall 2021

Current Program Replacement

Indicate if this new program will replace an existing program. If so, please list the program that will be replaced.

Certificate Programs

List whether or not this new program should be submitted to the Department of Education to determine if it is eligible for federal financial aid. Any questions should be directed to financeaid@uc.edu.

Special Fees

Special Fee Requests must be submitted separately to the Office of Budget and Financial Services.

The code for the college and program—e.g., 15BAC

Academic Plan Code

The current code abbreviation provided in plan/major–award format—e.g., HIST–BA

Academic Sub–Plan Code

The current code abbreviation provided in plan/major–sub-plan/track format—e.g., HIST–AH

Last Admit Term

Enter a term here—e.g., Spring 2021—if your intention is to close the Academic Plan or Sub–Plan.

Abbreviated Title

Indicate a new abbreviated title if you would like to change how a current abbreviated title is displayed. This must be 30 characters or less.

CIP Code

Identify the six–digit code that best applies to the program from the IPEDS Classification of Instructional Programs site. The CIP code drives the classification of the program and degrees reported to ODHE and USDE.

Length of Academic Program

Indicate the length of the program if it is being changed—in years. Must be a single digit; a range is not acceptable.
