Q:   CAN I WAIVE 2012 -13 UC STUDENT HEALTH INSURANCE?
A:   IN ORDER WAIVE UC STUDENT HEALTH INSURANCE  YOUR POLICY  MUST:

  • Contain no more than $1,500 annual deductible;
  • Contain at least a $500,000 lifetime maximum benefit;
  • Not contain a per incident or event maximum;
  • Not contain a per day policy maximum;
  • Not contain an inpatient or outpatient maximum (not including mental health care);
  • Allow at least 20 mental health visits per year and at least $15,000 for inpatient mental health care;
  • Contain only less restrictive limitations than the U.C. Student Health Insurance policy;
  • Be through a U.S. based insurance company employing a U.S. based claims administrator;
  • Be active the entire time for which you are enrolled in classes.

Any student who is eligible to receive medical services at the (VAMC) and who wishes to waive UC Student Health Insurance with that specific information, cannot waive online.

Submitting a paper waiver to UC Student Health Insurance will not fulfill the waiver requirement.  You must contact the Request of Information (ROI) department at your VAMC in order to sign a release and obtain an eligibility letter stating that you are eligible to receive medical services from the VAMC.  Once you obtain an eligibility letter, you must also fill out an official 2012-2013 paper waiver which can be printed from the link below.  You may also call 513-556-6868 to request that a paper waiver be sent or faxed to you. Prior to your waiver deadline, you must bring the eligibility letter and the paper waiver to the Student Health Insurance Office located in suite 334 of the Lindner Center or fax them to 513-556-6655.  A VAMC ROI department is located at room A47. The phone number is (513) 861-3100.  If this is a first time visit to the Vine Street ROI department, you are encouraged to call for directions to room A47 (ROI).

STUDENTS ENROLLED IN THE FOLLOWING PROGRAMS ARE NOT ELIGIBLE FOR STUDENT HEALTH INSURANCE:

Senior Audit

Professional Development

Dual Enrollment

Greater Cincinnati Consortium of Colleges & Universities

Being enrolled in a Distance Learning class and being enrolled in a Distance Learning program are entirely different enrollment statuses.  Any student who is enrolled in a Distance Learning program is not eligible for Student Health Insurance and will not be responsible for paying the Student Health Insurance premium.  There are no exceptions to this rule.
