Enrollment in one of UC’s voluntary benefit plans (critical illness and accident insurance) is handled through Unum’s Call Center (866-834-7821). Enrollment dates are Nov. 4-25.

For 2016, you will have a choice of two medical plans – the High Deductible Health Plan/Health Savings Account and the PPO.  Additionally, Anthem will replace Humana as the dental plan claims administrator. The plans and employee costs will remain the same in 2016.

Anthem’s Annual Enrollment Call Center is available to assist employees with questions about the transition from Humana Dental to Anthem Dental. Call the Dental Call Center toll free at 877-604-2156.  The Anthem Medical Customer Service Department can be reached at 844-249-5372.
