Immunization & Health Screening Requirements

Students living in shared housing are at higher risk for infectious diseases due to close contact and shared spaces. Vaccines are a safe and effective way to protect yourself and help support the health of the entire campus community. To help maintain a healthy campus environment, all UC students must comply with this University policy. 

The following immunizations are required:

  • Hepatitis B (3 dose) or HEPLISAV-B (2 dose)
  • Measles, Mumps, Rubella (MMR)
  • Meningococcal Quadrivalent (students 16-21 yrs. old)
  • Tetanus, Diphtheria, Pertussis (Tdap) 
  • Varicella (Chickenpox)

Tuberculosis Screening

Incoming international students and students in Health Science majors must complete a brief tuberculosis [TB] screening questionnaire.

Meningitis and Hepatitis B Information & Disclosure

By submitting the University of Cincinnati Immunization Form, students (or a parent/legal guardian for students under 18) disclose the student’s vaccination status for meningococcal disease and hepatitis B and acknowledge having read and understood information regarding the risks of these diseases and the availability and effectiveness of vaccines, as provided by the Ohio Department of Health , in accordance with Ohio Revised Code Section 3701.133.

Ohio law does not require vaccination; it requires disclosure. Submission of this form satisfies Ohio’s disclosure requirement for on-campus housing under Ohio Revised Code Section 1713.55Students who do not submit the required form are out of compliance.
