Important Information About Your New Anthem ID Card for Medical Benefits

Anthem Blue Cross and Blue Shield issues ID cards in the name of the subscriber (employee) only.

ID cards are not issued in dependent names nor do they include dependent names.

However, providers are familiar with this and the manner in which Anthem provides ID cards to members.

If an employee would like an additional card (with his/her name on it), he/she can contact Anthem at the customer service number shown on the card or temporary (paper) cards can be printed from the Anthem site (

www.anthem.com

).

Please remember that

Humana will continue as the university’s dental carrier

. New ID cards for dental coverage will

not

be issued unless an individual changed coverage levels.

Employees in the High Deductible Health Plan (with its Health Savings Account option) in 2014 received a Health Equity Transfer Request form over winter break. The form should be sent to Health Equity at the address shown on the form.
