Academic Fresh Start Application

To request an Academic Fresh Start, you will need to submit the application found below. The form will ask questions to verify that you meet the eligibility criteria.

1. You submit the Academic Fresh Start Application.
2. An email will be automatically sent to your college to verify the advising requirement.
3. After your college has verified that you have met with your academic advisor, you will receive an email notification, and the Office of the Registrar will review your application.
4. Once a decision has been made, you will receive an email notification to your UC email address.
